Im Gegensatz zu Levodopa ist es unabh\u00e4ngig von den pr\u00e4synaptischen Endigungen zur Speicherung und Freisetzung (<a href=\"#quellen\">Hagell et al., 2001<\/a>).<\/p>\n\n\n\n<p>Apomorphin ist eine stark lipophile Verbindung und die positive motorische Wirkung h\u00e4ngt von der Konzentration in der Gehirn-R\u00fcckenmarks-Fl\u00fcssigkeit ab. Aufgrund des hohen First-Pass-Effekts von Apomorphin ist die Bioverf\u00fcgbarkeit der oralen Formulierung sehr niedrig. Apomorphin ist eine Substanz mit einer hohen hepatischen Clearance, nur 3 &#8211; 4% werden unmodifiziert \u00fcber den Urin eliminiert (<a href=\"#quellen\">Neef, et al., 1999<\/a>).<\/p>\n\n\n\n<p>Nach subkutaner (s.c.) Injektion h\u00e4ngt die Absorptionsrate von der lokalen Blutzirkulation an der Injektionsstelle ab und betr\u00e4gt nahezu 100%.<\/p>\n\n\n\n<div style=\"height:40px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Wirkung von Dacepton<sup>\u00ae<\/sup> im Gehirn (englisches Video):<\/h2>\n\n\n\n<div style=\"height:30px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<figure class=\"wp-block-embed is-type-video is-provider-vimeo wp-block-embed-vimeo wp-embed-aspect-16-9 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\nhttps:\/\/vimeo.com\/652011230?share=copy\n<\/div><\/figure>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-theme-section alignfull\"><div class=\"wp-block-theme-section__content\">\n<h2 class=\"wp-block-heading is-style-main-title\" id=\"warnhinweise-und-vorsichtsma\u00dfnahmen\">Warnhinweise und Vorsichtsma\u00dfnahmen<\/h2>\n\n\n\n<p>Bei Patienten mit Nieren-, Lungen- oder Herz-Kreislauf-Erkrankungen sowie bei Patienten, die zu \u00dcbelkeit und Erbrechen neigen, sollte Apomorphinhydrochlorid-Hemihydrat nur mit Vorsicht angewendet werden.<\/p>\n\n\n\n<p>Bei \u00e4lteren und\/oder geschw\u00e4chten Patienten ist zu Beginn der Therapie besondere Vorsicht geboten.<br>Da Apomorphin selbst bei Vorbehandlung mit Domperidon zu Blutdruckabfall f\u00fchren kann, ist bei Patienten mit vorbestehenden Herzkrankheiten und bei Patienten, die mit vasoaktiven Arzneimitteln, wie z. B. Antihypertensiva, behandelt werden, Vorsicht geboten, vor allem bei Patienten mit vorbestehender orthostatischer Hypotonie.<\/p>\n\n\n\n<p>Da Apomorphin insbesondere in hoher Dosis das QT-Intervall verl\u00e4ngern kann, ist bei der Behandlung von Patienten mit einem Risiko f\u00fcr Torsades de Pointes Vorsicht geboten.<\/p>\n\n\n\n<p>Apomorphin geht mit lokalen, subkutanen Nebenwirkungen einher. Diese lassen sich in manchen F\u00e4llen durch Wechseln der Injektionsstellen oder m\u00f6glicherweise auch durch den Einsatz von Ultraschall (sofern vorhanden) reduzieren, um Bereiche mit Kn\u00f6tchen und Verh\u00e4rtungen zu vermeiden.<\/p>\n\n\n\n<p>Bei mit Apomorphin behandelten Patienten wurde \u00fcber h\u00e4molytische An\u00e4mie und Thrombozytopenie berichtet. Wie bei Levodopa sind bei gleichzeitiger Anwendung von Apomorphin regelm\u00e4\u00dfige Blutkontrollen angezeigt.<\/p>\n\n\n\n<p>Vorsicht ist geboten bei der Kombination von Apomorphin mit anderen Arzneimitteln, insbesondere solchen mit geringer therapeutischer Breite <a href=\"#quellen\">(siehe Abschnitt 4.5, SmPc in g\u00fcltiger Fassung)<\/a>.<\/p>\n\n\n\n<p>Bei vielen Patienten mit fortgeschrittenem Morbus Parkinson bestehen gleichzeitig neuropsychiatrische Probleme. Es gibt Hinweise darauf, dass neuropsychiatrische St\u00f6rungen bei manchen Patienten durch Apomorphin verst\u00e4rkt werden k\u00f6nnen.
